If you have answered yes to any of these questions apply for the role of Band 5 Reviewing Nurse in the Complex Care Team. We are currently looking for Band 5 registered nurse/s (RMN/RNLD) to join our team based at Norwich Union House in Huddersfield on a Full time basis. The working hours are 9am-5pm Monday to Friday with a mix of flexible working arrangements.

This team is responsible for working with partners in the wider MDT to support discharge of individuals who are currently placed in acute mental health/PICU and Locked Rehab wards. The team also review the NHS funding contribution of S117 Aftercare Packages of patients living in the community., The post holder will:

Assist the ICB in delivering its commissioning responsibilities as required in the Mental Health Act and code of practice for s117 Aftercare and the Who Pays Guidance.

Undertake clinical assessments and reviews of s117 aftercare funding eligibility for adults of all ages utilising the s117 Aftercare Matrix

Work with professionals, providers, individuals and their families and/or representatives to make determinations regarding NHS funding responsibilities for s117 Aftercare need requirements for new and existing service users.

Support in monitoring the standards of care delivered by care providers, giving advice and guidance to care staff regarding individual service users care needs in line with clinical governance.
