We are seeking a Health Care Support Workers to join York Community Response Team - to deliver high standards of interventions to patients within their own place of residence to prevent avoidable admissions to acute care and to support discharges. This role will be delivering interventions which support activities of daily living and promoting independence, as able.

Our well-established team covers the historical city of York as well as the rolling countryside within South Hambleton and Ryedale areas. The team consists of Physiotherapists, Occupational Therapists, Nurses and Support Workers and works alongside Primary Care and Partners to provide Urgent Care Response, Frailty Hub as well as short term reablement/rehabilitation programmes.

Do you feel ready for a new challenge and to make a difference to others alongside a fun, supportive and friendly team?PLEASE NOTE WE ARE UNABLE TO PROVIDE SPONSORSHIP FOR THIS ROLE., You will undertake specific duties delegated by the nursing, physiotherapy and occupational therapy team and will be responsible for providing holistic, person centred, compassionate health care to patient's 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patient's home.

We are looking for people who are kind, compassionate, creative, problem-solvers with excellent communication skills to join our expanding team.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy.

We provide a seven day service, between the hours of 8am - 8pm; therefore, successful candidates will take part in a rota that covers the hours of the service; however, we do offer a flexible and varied shift pattern.
