Full Job Description

A rare and exciting opportunity has become available for a Senior Band 6 Occupational Therapist to join the R.E.A.C.T. (Rapid Early Assessment and Care Team) team at Stoke Mandeville Hospital to cover Maternity Leave.

The successful candidate will be joining an established Therapy led Assessment and Discharge team which covers the Emergency Department, S.D.E.C (Same Day Emergency Care), A.M.U. (Acute Medical Unit) and the Short Stay Ward.

The REACT team provides very high quality assessments in order to establish robust and swift rehabilitation and discharge plans for the patients under their care. The team is made up of Physiotherapists, Occupational Therapists, a Nurse and Band 4 Clinical Support Workers and we work transprofessionally for the benefit of the patient. We also work very closely with the Discharge Co-ordinators, Hospital Social Workers, Speech and Language Therapists and Dieticians.

This is a very friendly, supportive and dynamic team with regular integrated individual supervision, appraisal, CPD, use of outcome measures and in service training.,

  • To participate within a clinical trans-professional team based in the A&E Department and short stay wards and contribute to its function.

  • To undertake assessments of patients with complex and diverse presentations taking into account their social needs outside of hospital.

  • To undertake advanced, comprehensive, and where applicable, standardised assessments clinical areas stated.

  • To participate within simple and complex rehabilitation and contribute to its function.

  • To use clinical reasoning skills and knowledge of evidence based practice to determine treatment plan using a wide range of treatment skills and options.

  • To analyse and interpret clinical and non-clinical information to form an accurate plan for a wide range of conditions, with guidance from Therapists within the team and the MDT when appropriate.

  • To use clinical knowledge to formulate problem lists and decide subsequent treatments, goal plans and selection of outcome measures.
