Full Job Description

Overview of Responsibilities Clinical works as part of a MDT within the PCN; consults with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans; evaluates and analyses clinical problems using their clinical knowledge, seeking out and applying relevant evidence, enhanced techniques, interventions, and equipment to make clinical decisions; delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and their underpinning knowledge to manage complex interventions; teaches and advises patients and their families on how to manage their condition or support the multi-disciplinary team to do so; participates in clinical audits and research projects and implements changes as required, including the development, and updating of practice protocols / guidelines and procedures locally;
works within national and local protocols where these exist; recognises boundaries of their practice and know when and to whom patients should be referred; using the principles of delegation, delegates work to other members of the MDT and take responsibility for the delegated activity as appropriate; demonstrates initiative and is creative in finding solutions to problems; holds responsibility for team performance and service delivery; provides supervision to PCN trainee nurse associate, PCN nurse associates and general practice nurses; facilitate and support the development and delivery of innovative training and education to staff, to improve competence and confidence, in line with best practice and guidance related to area of expertise participate in and/or lead formal training events promoting best practice in area of expertise; provide professional and clinical leadership and mentorship, acting as a clinical role model on the delivery of evidence-based practice; advise and support on
the development and delivery of work streams relating to area of expertise as part of the PCNs strategy, where appropriate; work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledge; contribute to the delivery of local policy, procedures and protocols for the PCN to improve identification, assessment, ongoing support for patients and their family carers. Regulatory Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning. Adhere to the Practice/Network Policies and Procedures including Infection Control, Health and Safety Be familiar with, actively promote and work within the spirit of the network practices Equal Opportunities Policy at all times. Be aware of their responsibilities under the Data Protection Act 1998 and abide by the eight principles of
that Act.

Any breach of the Act could result in disciplinary action being taken and criminal charges being brought against the individual. Personal / Professional Development Maintain own personal / professional development plan and portfolio thereby meeting the requirements for revalidation through appraisal of own performance and discuss development needs with mentor, supervisors and managers, on a regular basis. Participate in regular supervision, mentorship and training with other senior PCN nursing staff within the PCN complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles. Maintain NMC registration and professional development in line with expectations of the role Work at all times within the NMC Code of Professional Conduct.

Provide Mentorship and training of new recruits or students as and when required. Participate in the Practice/Network Performance Review process.
