CHC Nurse Assessor - RGN

The North Nottinghamshire CHC delivery team supports the facilitation of CHC services in Mansfield, Ashfield, Newark and Sherwood. Staff working within the ICB are expected to have strong links with internal and external teams across this patch as well as the wider ICS. All individuals will have an identified base, but travel for assessments is expected as part of the role. Currently, the ICB are operating a hybrid working model allowing a mix of office and home working to support colleagues to achieve a better work/life balance.

We are passionate about our role in high-quality support to people who access our services., The post holder will ensure that the review of the health care needs of individuals referred for continuing health care (CHC) eligibility are identified and met in accordance with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (FNC)
The post holder will collate and undertake comprehensive and coordinated assessments and decision-making for NHS Continuing Healthcare and NHS-funded nursing care with clients, carers and colleagues within health and social care. The post holder will also care plan and commission services to meet the needs of eligible clients.
The post holder will ensure timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost-effective.

This position is full-time with working hours of Monday to Friday 9-5.

The candidate will need to work in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets., The successful candidate will need to be able to demonstrate knowledge, skills and experience of person-centred assessments. The role will involve working within a multi-agency environment to support the assessment and review process for NHS Continuing Healthcare (CHC) NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets.
Experience undertaking mental capacity and best interests assessments is advantageous and an understanding of deprivation of liberty safeguards and the Court of Protection would be beneficial to the role. You will also need to be knowledgeable in report writing to ensure you can confidently complete a health needs assessment, personal care and support plan and the CHC Decision Support Tool.
You will need to be competent in using Microsoft Word, Excel and Outlook and also be confident in learning new systems for recording and report writing.
